Infant Dies From Accidental Overdose At Seattle Children’s Hospital

Posted on Oct 24, 2010

An infant has died at Seattle’s Children’s hospital after an accidental overdose. This is the third drug overdose death at this hospital in the last three years.

According to the Seattle Times, infant Kaia M. Zautner was given ten times the appropriate dose of calcium chloride by an intensive care nurse. The baby, who was eight months old, died ten days later from the medical error. The Department of Health is investigating the medication error death, though the hospital has taken full responsibility for the deadly mistake. In fact, the hospital publically admitted the death was their fault and told the media that coming forth, saying you are sorry, and fully investigating the incident would prevent future tragedies.

The hospital said that a mathematic miscalculation was the specific cause of the fatal medication error.

Last year, 15-year-old Michael Blankenship was killed by a Fentanyl patch overdose after a simple dental procedure at the same Washington State hospital. In that case, too, Seattle Children’s Hospital released a statement promising to end such medical errors. Blankenship’s mother has since filed a medical malpractice lawsuit against the hospital and many are expecting a similar lawsuit from Zautner’s parents in the coming months.

It is unclear whether changes took place at the hospital following the death of Blankenship 18 months ago.

Two weeks after this incident, the same hospital reported two other medical mistakes, both involving the incorrect administration of a drug. One medical error may have resulted in a second infant’s death, though that case, too, is still under investigation.
